Are operatives protected from cave-in and loose rock or soil that could roll into the excavation?
Are spoils and equipment set back at least 0.6 meters from the edge of the excavation?
Have adequate signs posted and barricades provided?
Has training as a Toolbox Talk been conducted with operatives prior to entering the excavation?

Have utility locations been reviewed with the operator and precautions taken to ensure that contact does not occur?
